This note is regarding Rehab hospitals and their suitability for Kaiser patients. Unfortunately I have had to have experiences in 3 different Rehab hospitals: The Rafael, Kindred of South Marin and my current one, Petaluma Post Acute Rehab (PPAR). In both of my previous stays I was there for several weeks thus I was able to accurately assess their strengths and weaknesses.

I would recommend, in the strongest possible terms, that Kaiser utilize as far as possible the PPAR over any of the other Rehab hospitals. There literally is no comparison possible. PPAR has superior PT, nursing, food, care and most undefinable, attitude of the workers. As an example at the other Rehabs, there was never a time that I had response less than 5 minutes to a call for help. At PPAR it is NEVER more than 5 mins. I’ve no space to elaborate but would be happy to answer questions.

It seems that your positive working relationship funnels down and is in part responsible for creating a healthy, supportive working environment for the nurses and other staff.

I feel very lucky to have the opportunity to percept at Petaluma Post Acute. I got more hands on experience than many of my classmates working in hospitals and feel better prepared because of it. I also had the opportunity to see how your facilty functions as a student nurse.

Two of the things that have attracted me to apply to Petaluma Post Acute Rehab are the level of excellence exhibited by the nursing staff, as well as their willingness to help each other out. As I mentioned before, I believe that a part of the pleasant work environment is due to the efforts to hire pleasant people and emphasize zero tolerance for backbiting.

I graduated last Thursday and am eager to pass my NCLEX and become part of your amazing staff. My husband, Robert has now been at Petaluma Care & Rehab for almost 8 months. During that time his care has been excellent, his health has improved and our evaluation of the facility would be that it is very good. Bob appears to have no complaints about the food, his personal care or attention to his individual health needs. He has received therapy for getting “back on his feet”. I think the activities department does an excellent job of providing entertainment for the patients. We enjoy being able to so easily go outside to capitalize on the fresh air, sunshine, and beautiful view.

My assessment of Petaluma Care & Rehab. would be that it is a very fine facility with caring and knowledgeable staff members. Concern for comfort and cleanliness and ultimate interest in the well being of each patient.

It has been my good fortune to recuperate from hip replacement surgery at Petaluma Post-Acute Rehab over the last 17 days, thanks to Kaiser Permanenete and Medicare.

I left yesterday and will miss the excellent care which I received during my stay, both physical therapy and nursing staff provided excellent personal care. My needs were anticipated by caring staff with an attitude of positive success for me, especially at those times when recovery seemed so far away.

I experienced the skills of a caring and talented staff which worked hard under all conditions.

I learned a lot about patient care during my 17 day stay. Night or day staff responded quickly and willingly in order to help me. Today I am able to walk with a walker again and my hip is almost happy.

We want to express a special note of appreciation to Tara, LVN. It was clear to the family, she took extra care to help.

A special thanks to all staff from the administration, to housekeeping and the kitchen. Why are there not more such rehab facilities? Petaluma Post-Acute Rehab is a great pioneer.

My 93 year old Grandma was a resident at Petaluma Post Acute Rehab from November 2012 until her death in February 2013. Not only did the staff take great care of my beloved Grandma, but they went above expectations in supporting me.

The nurses and CNAs really made a difference in my Grandma’s last months. One nurse in particular was her absoute favorite. Just by being on duty in Grandma’s wing calmed her. When he wasn’t on duty one day, Grandma seemed sad. When I asked what was wrong, she said “Oh, I guess it’s because Doug isn’t here.”

The other nurses were also helpful, compassionate and caring. Everyone took the time to listen to the patient and family. The CNAs made sure Grandma was comfortable and clean, and did so gently.

The food was healthy and flavorful, not that watered down no-seasoning stuff you get at other places. The nutritionalists came by often to make sure that the food was good, and that she received the food she wanted and that was best for her.

Social services made sure that even though Grandma was unable to get out of bed that she had activities that she liked. They arranged for a volunteer to come by to visit and read with her, who ended up being the daughter of a friend – that’s what you get in a small town!

The admittance people were so accommodating. Every little request was met with a smile and done promptly. Housekeeping was thorough and also made sure that everything was always clean.

This is all possible because of the leadership provided by the Administrator.

I have a loved one at Petaluma Post Acute Rehab, my mother.

She is 87 and has been a resident for almost three years.

My mother has always battled depression and more recently has dealt with injuries and illness and now also battles Parkinson’s. She is unable to stand, has difficulty feeding and taking care of herself in ways we all take for granted.

Since my mother has been at Petaluma Post-Acute Rehab, I have watched her bloom. She joins in all the activities, her anxiety is much less and her mind is so much better. She has friends in the staff and other residents.

I am very lucky to know that when we are not able to be there, that she has people caring for her who know her and really do care. Who will also alert us of her needs or any changes.

Not only is she cared for and safe, but since my mother has been at Petaluma Post-Acute Rehab, I have watched her bloom. She joins in all the activities, her anxiety is much less and her mind is so much better.

When someone you love is hurting or sick, sometimes it’s hard to leave them alone, but I have to say that at Petaluma Post Acute Rehab, I NEVER felt like my mom was left alone. She was always clean, her needs were met and when she finally left, she left to hugs and cheering and feeling of support and caring that was priceless.

I am grateful beyond words for this facility. The minute I walked in, I knew this place was different. It is warm and inviting and as close to home as one could feel in a place like this. I was a patient here needing rehabilitation and wound care after surgery and major injuries initially treated at Santa Rosa Hospital.

The nursing, PT and CNA care was excellent. The food was very good. Everyone was extremely attentive and pleasant.

The facility is very attractive, in a private setting, with a nice outdoor area.

I felt very well cared for and responses to my requests were timely and cheerful

I don’t live in the area, but if I did, I would most definitely highly recommend this facility. It was a pleasure staying here and interacting with an outstanding staff.

My grandmother at the age of 86 recently broke her hip and it was a rather traumatic event for my family and I. Once she had her surgery we quickly were looking for a place where she could recover in dignity. Fortunately the team from Post-Acute Rehab in Petaluma were close by and immediately met my mother who was with my grandmother at the hospital to outline the rehab services that were available to my grandmother. We ended up sending her to their facility for her rehab process and couldn’t be more pleased with the care she has been given.

I am writing to commend the culinary staff for their high degree of skill and commitment to service for patients and residents.
First, let me say that it is a very difficult task for every part of your organization to provide care to the highest professional standards. It is difficult because you all deal with a wide variety of medical and personal issues in a large and every changing population of patients and residents. Second, in order to deliver the highest quality of service, requires the effort of many different parts of your organization. Those many different moving parts must work in harmony for your organization to fulfill its service mission. Third, for the benefit of everyone, it is important that excellence in the discharge of duties be recognized and commended by those served by your organization.
Against the background, and on behalf of my wife, Toni, and myself, I wish to express our thank you for the high quality of food and service delivered by the culinary staff. It is deserving of recognition. They perform extremely well a function that is very important to resident and patient satisfaction. Over many years, Toni and I have eaten in many fine restaurants all over the U.S. and Europe so we are not easy to please. It is with this in mind that I also use the word “culinary” purposefully and carefully. The staff merits that title.
I have tasted virtually every food item delivered by the staff to Toni and uniformly find them to be of high quality and tasty. Toni has not refused to eat any entrée or side dish. Quite the contrary, I can tell the way she eats with gusto that she genuinely likes the food very much. It looks very good and it tastes very good. The staff also adheres to very high standards of service. If something does not suit someone’s taste buds, accommodation for either a single meal or for meals on a regular basis, is promptly and cheerfully made.
The culinary staff makes an important and positive contribution to a successful recovery. I think that people who eat well tend to recover well. We are very thankful for their efforts. We commend them on important work well done.

I wanted to write these letters to thank all those of you who work hard to run the the Petaluma Post-Acute Rehab Center. This kind of nursing care is needed by so many. After working in the medical field when younger, now at 76 I am experiencing the “other side”.

Last month while I was there for 2 weeks, many were very kind to me. Those days after surgery would have been extremely difficult without their help. I have also sent little “thank-you” cards to those nurses whose names I could remember.

After returning home, the Kaiser Home Health sent nurses to my house. They monitored my medications until I was off pain meds and more clear headed. Physical Therapy was continued for a time as well.

They were all very kind and helpful so sent them a “thank-you” card.

I am healing very well and getting stronger each day. Gradually becoming more confident. Slowly “getting on with my life.” Even interested again in my letter writing that I do. It’s one of the things I’ve always enjoyed. This is possible for me during these days of having so much sit-down time,

I was admitted to Petaluma Post Acute Rehab on April 1, 2016 and I will be discharged tomorrow, June 10. I’m writing to inform you how impressed I am with the excellence of the nursing, CNA, and rehab staff members. In all my interactions with them, they have been patient, compassionate, knowledgable and professional, and willing to go out of their way to do things for me. To give you specific examples, one staff member brought me cinnamon because I told her I liked it on my morning oatmeal, another brought me tea in the early morning on the days she worked, several CNA’s have gone out of their way to take lunch requests to the kitchen, to recommend food items, to bring me ice cream, or to track down clothing that was misplaced. They have made my stay here a better experience than I was expecting, and I believe that their attitude and dedication have promoted my healing and my return home. I’ve seen the box ask for nominations for ‘Five Star’ staff members. I’ve considered nominating someone, but I was reluctant to single anyone out because everyone has been such a pleasure to deal with. I commend this facility for the quality of their staff members and thank everyone for their help with my recovery.

I wanted to take a moment and express my (and my family’s) appreciation for the care recently provided to my father, Hal L. Although I live in the Seattle area, I was fortunate to have the opportunity to spend time in Santa Rosa with my parents, visiting my father nearly every day for the three weeks that I was in town and he was an inpatient in the Kaiser San Rafael medical center and your facility (I had to return home to my immediate family before he was discharged to home).

For background purposes, I spent the vast majority of my career working with major healthcare providers across the western United States.  The bulk of my time was spent working in the acute care environment where I had the opportunity to better understand the challenges faced by healthcare providers.  As such, I have a great appreciation for all “the moving parts” that you, your management team, and your staff deal with daily.  You know better than I that there is not a “straight line” or a simple set of steps that move patients to wellness or a state where they can be safely discharged. “Things” happen and patients are not always in-step with the team that is charged with caring for the patient.  Recovery periods, especially for the elderly or those with cognitive challenges, can take longer periods of time.

In spite of all those challenges and obstacles to success, I was most impressed with the care that was provided to my father.  Over the course of my visits to your facility, I had the chance to observe your team and engage with various staff during my visits.  I am sincerely grateful to Petaluma Post-Acute Rehab for the care provided to my father and the other patients.  Although my memory is not as strong as it used to be, I would like to acknowledge the staff that I engaged with on an ongoing basis.  I am certain that I have lost track of some of the names of the staff that I met with and I will fail to list their names in this note.  Sorry about that. Please do not misinterpret that “miss”; it’s my memory.

As we prepared for my father’s discharge from his inpatient stay at Kaiser San Rafael, I had the task of finding a rehab facility that would provide great care AND be relatively close to my parents’ home in Santa Rosa.  Given the fires in Sonoma County, our options were limited in Santa Rosa as the facilities were full.  I was given the name of your facility as a candidate.  I checked out the CMS website and discovered that Petaluma Post-Acute Rehab scored very well.   I spoke to Laurie H and she convinced me that, in spite of the distance, your program was equipped to address the needs of my father.  She was very proud of your wound care services.  I concurred with Laurie and the journey began.   Jesus Camacho did a great job of facilitating the admission process and we moved my father to Petaluma.

Wound care was very important to my father’s situation and Cari, as well as Rachel, did an awesome job of attending to the daily needs of my dad’s wounds AND keeping me apprised of the progress that was occurring daily.   Cari was especially effective in keeping me apprised of the progress and assuring me that my father would not be discharged until the team concluded that it was safe to do so.  Your team of therapists (i.e. physical, occupational, speech) were very kind to my father, yet pushed him to get stronger and healthier.  I had the opportunity to observe Erin, Kim, and Susan engage with my father; nicely done.  Your nurses and nursing assistants are a special group of folks that clearly exhibited top-notch services in a loving, compassionate manner.  I engaged with Diana, Maria, Roger, Francesca, Vern, Jennifer, Elizabeth, Cathy, Erica, Eli, Ally, Zameka, and Matt, to name some of the folks that I met.  I know that I left off a couple of names (sorry), but you should know that you team did an awesome job of attending to the needs of my father as well as answering my endless series of questions.  In addition, Dietary and Housekeeping staff exceeded our expectations in terms of quality and services; silently going about their work and keeping the operations running smoothly.

It’s easy to understand why you score so high in CMS’ ratings as your team serves your patients well.

So, on behalf of our family, let again say “Thank You” to the team at Petaluma Post-Acute Rehab.  Please keep up your great work; our healthcare system needs more providers like you.  We wish you Happy Holidays.  Please pass on this note of appreciation to your team.  They are awesome.
